Please list your children and beneficiaries in birth order, and provide their full legal name as you would want it to appear in your estate planning documents. With regard to children: If you or your spouse have children from a prior marriage or relationship, please indicate which spouse is the parent. If you have a child who is legally adopted, please indicate the date the child was adopted. If you have a deceased child who left a surviving spouse or children, please provide the child’s date of death and information for the surviving spouse/children.

If you have personal property of significant value, for example, an insured art collection, indicate your best guess at value. We do not need information on “typical” personal property. Automobiles that are owned jointly will indicate “and” or “or” on the title.
